Crystal Hazelton, PT, MPT, OCS, PRPC
Crystal Hazelton is a movement specialist and the founder of COVA Physical Therapy. With professional training as a physical therapist and dual board certification in both Orthopedics and Pelvic Health, she delivers expert, whole-body care that empowers clients to move better, live stronger, and thrive at every stage of life.​

Her early work with the Navy SEAL Human Performance team shaped her belief that elite-level care should be accessible to everyone—not just tactical or professional athletes. She also brings extensive performing arts experience to her practice, having worked backstage on six national Broadway tours through Neurotour Physical Therapy, helping dancers and performers stay injury-free and performance-ready.

Today, Crystal blends science, precision, and compassion to support active women, competitive dancers, female athletes, and frontline professionals. She specializes in bridging the gap between rehabilitation and high-level performance through a personalized client-focused approach rooted in real results.

Crystal was honored as a 2025 Woman of Impact by Assemblymember Tasha Boerner for her contributions to postpartum recovery for Active Duty Sailors. She was also awarded the Master Clinician title at Naval Medical Center San Diego in recognition of her clinical excellence.

With advanced training in orthopedics, sports performance, pelvic health, and menopause, Dr. Annalise goes beyond symptom relief. She identifies the root cause and creates a plan tailored to your body, your goals, and your lifestyle.

Her sessions combine precise exercise, manual therapy, and alignment-focused training to restore strength, improve control, and prevent future injury—whether you’re returning to workouts, staying active through life transitions, or pushing your performance to the next level.
